Hospital & Clinic Registration Information: Pre-registration is now closed. You can register onsite at the event.

Exhibitor Registration: Pre-Registration is now closed. You can register onsite as a General Sponsor. If you are already a Sponsor or Exhibitor, but need to make changes to your attendees or add an additional attendee, you can do this onsite when you arrive for set-up/check-in.

Vendor/Booth Shipping Instructions: Please click here for shipping label and instructions. Please note anything that arrives three (3) or more days prior to the conference will have a storage fee; this adds up quickly per box/per day.
Vendor/Booth Electrical Order Form: If you need to order electricity or any other supplies for your booth, please complete this form. You will work directly with the hotel for your purchases. If you purchased a booth package that comes with electricity, you do not need to complete this form. TORCH will handle the electrical purchase on your behalf.
